Active will provide free practical support and guidance to all patients and can assist with issues such as:
- Helping you understand your diagnosis and prognosis and the medical terms being used
- Assistance with navigating the healthcare system – both NHS and private – to ensure you receive the treatment you need as soon as possible.
- Advising about care after your discharge from hospital
- Recommending equipment you might need and how to access it
- Connecting you with medical specialists in your area
